This learning day is specifically for enhancing the capability and capacity of the LTCC Programme teams (programme managers, executive sponsors, information managers, service improvement & clinical leads).  To ensure links we welcome key partner agency leads (e.g. rehab coordinators, fall leads, pharmacy, voluntary sector, social care partners).

Outcomes:

  • Approach focused on patient-centred care for people with long term conditions
  • Tangible involvement for people with LTCs and their carers
  • Effective staff and multi-agency engagement
  • Effective communication with patients and carers

Confirmation details will be issued 7-10 days before the event. SHSCEvents cannot be held responsible for non-arrival of registration information. If you have not heard from us 7 days prior to the conference, please contact us on 0131 275 7751. Many of the events that the Improvement and Support Team provide are oversubscribed and we actively manage waiting lists to ensure that we use all the capacity that we have available. We understand that due to the nature of health service work things can happen unexpectedly, so we ask that staff cancel in time for us to reallocate places to people on the waiting list. The Improvement and Support Team are now placing a 48 hour cancellation rule on all of their training and development events. Failure to notify SHSCEvents within the aforementioned timescale will result in a £50 administrative fee being applied.
